Activities to engage persons for the purpose of providing immediate support and intervention, as well as identifying potential program participants
Street Outreach activities are designed to meet the immediate needs of people experiencing homelessness in unsheltered locations by connecting them with emergency shelter, housing, or critical services, and providing them with urgent, non-facility-based care.
Essential services related to reaching out to all unsheltered individuals and families experiencing homelessness with the CoC’s geographic area, including those least likely to request assistance. Services include connection to emergency shelter, housing, critical/crisis services, and urgent, non-facility-based care.

Street outreach is a direct, person-centered approach where trained staff engage with individuals and families experiencing homelessness—particularly those living unsheltered, in cars, encampments, or public spaces. The goal is to build trust and connect people with the support they need.

Outreach teams support:
Individuals and families living on the street
People in cars, tents, or abandoned buildings
Those not connected to services or unsure where to go
Individuals at risk of harm due to weather, violence, or health conditions

Basic supplies like food, hygiene items, and blankets
Housing referrals and case management
Assistance with ID, benefits, or paperwork
Transportation to appointments or shelters (when possible)

No. Street outreach is about building trust and long-term relationships. Staff meet people where they are—physically and emotionally—and support them at their own pace, without judgment or pressure.

Staff travel throughout the community, visiting known encampments, responding to referrals from local partners or residents, and keeping regular contact with people they’ve engaged before. Outreach is ongoing and consistent.
