Shelter Plus Care is a Supported Housing Program that provides 10 units of permanent housing and includes case management support to homeless individuals and their families that are diagnosed with a Severe and Persistent Mental Illnes (SPM). This comprehensive program aims to increase the independent living skills and overall stability of all participants.

Interested applicants must apply through the Dutchess County Department of Mental Hygiene's Single Point of Entry (SPOE) process. All residents of HRH's S+C Program must be:

  • Able to handle personal care needs
  • Able to self-medicate
  • Willing to commit to a service plan
  • Willing to participate in an educational or vocational program

Though anyone satisfying the above criteria is encouraged to apply, preference will be given to individuals from Dutchess County over residents of other counties.

According to the Shelter Plus Care program, applicants are defined as "homeless" if they are coming from one of the following:

  • The streets (i.e., living or sleeping in the streets, in park, or in an abandoned building)
  • An emergency shelter
  • A transitional housing facility for homeless people

Supportive services are offered by Hudson River Housing in collaboration with the Dutchess County Department of Mental Hygiene. Case workers assess the specific needs of each client and the factors contributing to their homelessness. They work with the client to develop a service plan and assess the client's commitment and progress toward achieving the planned goals.

HRH also assists the clients toward these goals by providing access to an array of community supportive services, including: support groups, mental health services, and skills development training (e.g., financial management, job training courses).
