Long Term Housing ServicesMaximize - 12 units of low cost housing (two and three bedroom apartments) for homeless families. Supportive services are provided. Shelter Plus Care - 10 units of rental housing serving homeless adults, who have been diagnosed with a severe and persistent mental illness, and their families. Supportive services are provided. Home Base I & II - 12 units of rental housing for homeless young adults who are in treatment for chemical dependency, and their families. Support services are provided. COACH - Eight units of rental housing for homeless individuals who have been diagnosed with severe and persistent mental illness or chemical dependency and are employable. Support services are provided. Transitional Housing ServicesOur Transitional Housing programs offer safe, supportive housing and services as participants gradually develop the skills and means needed to transition to self-sufficient living. River Haven Transitional Living Community (TLC) : A ten-bed, supervised program for 16 to 21 year old homeless youth. Length of stay cannot exceed 18 months. Support services are provided. Hillcrest House : 58 single room occupancy (SRO) units for homeless adults. Residents can live at Hillcrest House for up to 24 months. Meal plans and support services are provided. La Grange House - LaGrange House case management services based out of 313 Mill Street, will serve formerly homeless individuals and families whose present living conditions are not permanent. Staff will assess the client's strength's weaknesses and ability to obtain and retain permanent housing. At that point new housing will be sought. Emergency Shelter ServicesDutchess County Coalition for the Homeless (DCCH) - A 60-bed overnight shelter for homeless adults. Dinner, breakfast, shower and laundry facilities are provided, along with with advocacy and referral services. Gannett House - The Gannett House Program, based out of 313 Mill Street in Poughkeepsie, offers case management services to individuals and families placed in emergency housing who seek a quick transition to independent living. Case Managers work on a variety of issues, in a variety of settings, all the time working to locate the housing their clients desire. River Haven Shelter - A 12-bed shelter for homeless and/or runaway youth, ages 10 through 17. Meals, laundry facilities and supportive services are provided. AHEP - After Hours Emergency Placement - Formerly coordinated by the Red Cross, in 2002 HRH took responsibility for placing all Dutchess County individuals and families that became homeless outside of regular business hours due to non-disaster related circumstances.
